Bring the following original documents and certificates to your interview:

  • AHCA Background Screening
  • Certified Nursing Assistant/Home Health Aide Certificate
  • Physical Exam and TB Test
  • Proof of Citizenship
  • Social Security card
  • Driver’s License
  • Car Insurance
  • Car Registration

Current certificate for the following trainings:

  • CPR card
  • HIV/AIDs
  • OSHA/BBP/TB/Infection Control
  • Alzheimer’s
  • Domestic violence
