Ideal candidate is customer service focused. This position is to staff caregivers to clients needs

  • Assists the Case Managers to promote the delivery of coordinated home health services by conferring with the members of the home health care team, as assigned, receiving patient referrals and ensuring a licensed professional has completed the initial patient assessment.
  • Obtains authorizations for care and facilitates client’s plan of care.
  • Assists and monitor the delivery of patient care services including:
  • Ensuring the frequency of services delivered coincides with the plan of care;
  • Monitoring patient and family satisfaction with services delivered;
  • Monitoring the activity of the client care staff
  • Ensuring all care and services are provided pursuant to the plan of care and with appropriate authorization, as may be required.
  • Participates in evaluating the effectiveness of the services provided in consideration of the desired outcomes. Contact clients to initiate services and schedule as directed. Continue ongoing communication with clients and/or family members to ensure the quality of care is being met.

Position Requirements:

  • Prior experience in health care, home care, staffing, recruiting or account management strongly preferred
  • Effective written and verbal communication skills
  • Familiarity with database management software programs and word processing.
  • Successful ability to negotiate conflict.
  • Effective organizational skills with the attention to accuracy, detail, follow-up.
  • On-call availability
