This innovative model is designed to enhance the lives of individuals with dementia while easing the burden on family caregivers by delivering coordinated, person-centered care in the home.

What Is the GUIDE Model?

The GUIDE Model is part of CMS’s effort to redesign dementia care in a way that prioritizes dignity, compassionate support, and better quality of life for individuals and their families. The program connects people living with dementia to a dedicated care team that helps manage care more effectively—whether it involves medical coordination, home support, or caregiver education.

Key elements of the GUIDE Model include:

  • Care Coordination: Each participating individual is assigned a dementia care navigator who helps streamline communication between doctors, care providers, and family members. This reduces confusion and helps ensure that medical needs and support services are aligned and timely.
  • Caregiver Support and Training: The GUIDE Model recognizes the central role of family caregivers and offers education and training to help them understand dementia progression, manage challenging behaviors, and maintain their own well-being.
  • Respite Services: To prevent caregiver burnout, the program offers access to respite care, allowing family members to take breaks while their loved one receives short-term support from trained professionals.

  • In-home respite care to give caregivers time to rest and recharge
