A tribute card is a heartfelt way to honor someone special while extending care and compassion to others. Whether you’re celebrating a milestone, offering condolences, or simply sharing gratitude, your tribute helps Jewish Family Home Care continue its mission of providing trusted, life-enhancing care for older adults throughout Broward County.
Each tribute supports our work, offering home care services to individuals of all faiths and backgrounds, including aging Holocaust survivors who helped build our community. Every card sent helps ensure that older adults in need can live safely, independently, and with dignity.

When you make a tribute donation, we’ll send a personalized card to your honoree or their loved ones, letting them know a meaningful gift has been made in their honor or memory. Your generosity brings comfort not only to the person you’re recognizing but also to those who depend on Jewish Family Home Care’s support each day.
How It Works
Tribute Form Instructions
- Choose One-time or Monthly gift.
- Select a predesignated gift amount or fill in a custom amount.
- Check the box: Dedicate my donation in honor or in memory of someone.
- Check “in honor of” or “in memory of”.
- Fill in the tribute card recipient information. Note: the Honoree Name and Recipient Name can be the same or different. The tribute card will be mailed to the person listed in the Recipient field.
- Enter a mailing address for the Recipient (the Tribute Card will be mailed to this individual).
- Write a personal Message for the recipient. This is optional, but can be customized for the occasion and to the person who is the recipient of the Tribute Card.
- If you have special instruction for the Jewish Family Home Care team regarding the Tribute Card, check the “Write us a comment” box and fill in the corresponding field.
- Choose Next.
- Enter your Information (name, email, address).
- Fill in your payment information on our secure platform and choose Donate.
